SER-Niños Charter School System

Dear SER-Ninos Families,
As part of compliance with Senate Bill 12, Hitchcock ISD must provide parents/guardians with information on their rights and collect written consent for services provided at school.

The new SB12 Consent Form is available here and must be completed electronically for each student by September 12th, 2025.

The form is a three-step process that covers:

1. Health-Related Services: This includes basic care, such as first aid (bandages, ice packs, etc.), temperature checks, and vision screenings.

2. School Counseling Services: This gives your child access to our Guidance and Counseling Department.

3. Extracurricular Activities and Clubs: Participation in school clubs and activities requires this.

👉 Important: All parents must complete this electronic form.
📌 Deadline: Previous paper consent forms will not be honored after the deadline. Starting Monday, October 6, 2025 , your child must have an electronic consent form on file to continue to receive these services.
